Only 34 (maybe a few more with DLC, anniversary) followers can be stewards in Hearthfire. This mostly overlaps with with GHP with a couple exceptions either way that I can't remember. This means many (most) of your potential followers simply cannot be stewards. For example the un-killable Mjoll is not on the Hearthfire list.In Falkreath, go to the Dead Man's Drink and into the owner's room where you will find a note entitled Mysterious Altar.Read it to start the quest.
